To request materials:
Send a note with your name, mailing address, home and work phone numbers, e-mail address, and the item you are requesting with a deposit check for $50 made payable to ADA-DPG 11. It's fine to request more than one item - after the first item is returned, the next one will be sent out, then the next, etc. Your deposit will be mailed to you when everything is received back in good condition.
Only one item is loaned at a time. All materials are loaned on a first come-first served basis. If material is out, your name will be placed on the waiting list. If the waiting list is longer than three months, you will be notified. Items must be insured, as instructed, and returned by Priority Mail.
